This de-identified image demonstrates nicely the importance of clinical work up for patients with pelvic pain. This patient was referred for fibroid management, however during our consultation the clinical suggestion was that ovarian vein incompetence was more likely to be the culprit of her symptoms. This work up CT beautifully ilustrates the large refluxing left ovarian vein (arrowed). The fibroids in fact were smaller and no longer receiving a blood supply therefore did not require treatment. This patient went on to have ovarian vein embolisation.
